2027 Part B and Part D premiums by income (IRMAA)

The 2027 brackets use modified adjusted gross income from your 2025 return. Thresholds are computed from the statute (see IRMAA brackets 2027 for the method and back-test). Part D amounts are computed with the statutory formula from CMS's final $41.33 base beneficiary premium. Part B dollar amounts are estimates at the $209.50 projection until CMS publishes its table.

Married filing separately (lived with spouse during the year): standard premium up to $112,000, the 80% bracket above that and below $388,000, and the 85% bracket at $388,000 or more. Where a threshold shows two values, the answer depends on how CMS treats the October 2025 CPI-U that was never published. In 2024 to 2026, multiplying the standard premium by the share of cost lands within 10 cents of CMS's table.

Medicare Part D in 2027: deductible, cap and IRMAA

Under the Inflation Reduction Act redesign, the standard Part D deductible rises to $700 and the annual out-of-pocket cap to $2,400 in 2027 (from $615 and $2,100). Both are indexed to per-enrollee Part D costs and were finalized in the CY 2027 Rate Announcement. Many plans charge a lower deductible; none may charge more.

The base beneficiary premium, which drives Part D IRMAA and the late-enrollment penalty, is $41.33 for 2027: the 2026 amount of $38.99 plus the 6% cap that applies through 2029. By statute, Part D IRMAA equals that premium times (applicable percentage − 25.5%) ÷ 25.5%; CMS rounds the result to the nearest 10 cents. That method reproduces every CMS amount from 2022 to 2026, so the 2027 row is computed, not yet published by CMS:

Medicare Part A in 2027

CMS announces the 2027 Part A deductible and coinsurance with the Part B premium. For reference, 2026 amounts are: a $1,736 inpatient deductible per benefit period, $434 a day for days 61–90, $868 a day for lifetime reserve days, and $217.00 a day for skilled nursing days 21–100.

About 99% of people pay no Part A premium because they have 40 or more quarters of Medicare-covered work. Those who buy Part A paid $311 a month (30–39 quarters) or $565 (fewer than 30) in 2026.

When will CMS announce 2027 Medicare premiums?

There is no fixed date. CMS has released next-year premiums anywhere from late September to mid-November. Check the CMS newsroom; we update this page and the IRMAA pages the day the 2027 fact sheet is out.

Will the 3.6% COLA cover the 2027 Part B increase?

The 2027 Social Security cost-of-living adjustment is 3.6%. On a $2,000 monthly benefit that is about $72 more a month, against a projected $6.60 increase in the standard premium, so the net payment still rises by roughly $65. Any benefit above about $184 a month gets a COLA larger than the projected increase.

The hold-harmless rule protects people whose Part B premium is deducted from Social Security: if the COLA in dollars is smaller than the premium increase, their premium rises only by the COLA amount. It does not apply to new enrollees, people paying IRMAA, or people whose premium Medicaid pays. With a 3.6% COLA and a projected $6.60 increase, it should matter to very few people in 2027.

Medicare Part B premium 2028: early projection

The same Trustees table (V.E2, intermediate estimates) projects a 2028 standard Part B premium of $224.50 and a deductible of $313, which would be $15.00 more than the 2027 projection. These are long-range estimates; the 2027 Trustees Report, due in 2027, will update them before CMS sets 2028 amounts in late 2027. 2028 IRMAA will be based on 2026 income, and the $500,000 and $750,000 top thresholds start rising with inflation that year.

Medicare 2027 questions

What is the Medicare Part B premium for 2027?

CMS has not announced it yet. The 2026 Medicare Trustees Report estimates the 2027 standard Part B premium at $209.50 a month, up $6.60 (3.3%) from $202.90 in 2026. The official amount usually comes out between late September and mid-November.

How much is Medicare going up in 2027?

On the Trustees projection, the standard Part B premium rises $6.60 a month, about $79.20 a year. That is smaller than recent increases: $17.90 for 2026 and $10.30 for 2025. The Part D deductible rises from $615 to $700 and the Part D out-of-pocket cap from $2,100 to $2,400; those two are final.

What is the Medicare Part B deductible for 2027?

Not announced. CMS sets it with the 2027 premium. The 2026 Medicare Trustees Report (Table V.E2) estimates $292, up from $283 in 2026 and $257 in 2025. Like the premium, that is a projection until CMS publishes.

What are the Medicare Part B premium 2027 income limits?

IRMAA for 2027 uses your 2025 tax return. Computed from the statute, the surcharge starts above $112,000 of MAGI for single filers and $224,000 for joint filers, and the top bracket starts at $500,000 and $750,000. Higher brackets pay 35%, 50%, 65%, 80% or 85% of Part B cost instead of 25%.

Will the 2027 Social Security COLA cover the Part B increase?

For almost everyone, yes. The 2027 COLA is 3.6%, which adds about $36 a month to a $1,000 benefit, far more than the projected $6.60 premium increase. When the COLA is smaller than the premium increase, the hold-harmless rule stops the net Social Security payment from falling for people who qualify.

What is the Medicare Part D out-of-pocket maximum for 2027?

$2,400, up from $2,100 in 2026. Once your covered drug costs reach it, you pay nothing more for covered drugs that year. The standard deductible is $700. CMS set both in the CY 2027 Rate Announcement on April 6, 2026.

When will CMS announce the 2027 Medicare premiums?

CMS released 2026 amounts on November 14, 2025, 2025 amounts on November 8, 2024, and 2024 amounts on October 12, 2023. Expect 2027 amounts between now and mid-November 2026. We update this page when they are published.
