Form 5498: IRA Contribution Information, Box by Box

Form 5498 is the IRA contribution information form: the IRS 2026 instructions have the trustee or issuer of an IRA file it with the IRS by May 31, 2027, for each person for whom it maintained an IRA in 2026, and provide each participant a statement by February 1, 2027. It reports contributions, rollovers, Roth IRA conversions, recharacterizations, the account’s fair market value and required minimum distribution (RMD) information, and the table below goes through it box by box.

What Form 5498 reports

The IRS About Form 5498 page describes who the form is filed for:

“File this form for each person for whom you maintained any individual retirement arrangement (IRA), including a deemed IRA under section 408(q).”
IRS, About Form 5498

The participant instructions printed on Copy B of Form 5498 (2026) describe what the form reports:

“The information on Form 5498 is submitted to the IRS by the trustee or issuer of your individual retirement arrangement (IRA) to report contributions, including any catch-up contributions, rollovers, repayments, required minimum distributions (RMDs), and the fair market value (FMV) of the account.”

“For a Trump account, Form 5498 is only used after the account beneficiary’s growth period. Form 5498-TA is used during the account beneficiary’s growth period.”

Copy B, the participant’s copy, carries this line near the top:

“This information is being furnished to the IRS.”

Who files it and who receives it

The Instructions for Forms 1099-R and 5498 (2026) put the filing duty on the trustee or issuer of the IRA, and give the filing date:

“File Form 5498, IRA Contribution Information, with the IRS by May 31, 2027, for each person for whom in 2026 you maintained any individual retirement arrangement (IRA), including a deemed IRA under section 408(q).”
IRS Instructions for Forms 1099-R and 5498 (2026), Specific Instructions for Form 5498

The instructions treat an IRA plan as one account for this purpose:

“An IRA includes all investments under one IRA plan. It is not necessary to file a Form 5498 for each investment under one plan. For example, if a participant has three certificates of deposit (CDs) under one IRA plan, only one Form 5498 is required for all contributions and the fair market values (FMVs) of the CDs under the plan. However, if a participant has established more than one IRA plan with the same trustee, a separate Form 5498 must be filed for each plan.”
IRS Instructions for Forms 1099-R and 5498 (2026), Specific Instructions for Form 5498

They also say the form is filed after distributions have started, and give a rule for spousal IRAs:

“Caution: You are required to file Form 5498 even if required minimum distributions (RMDs) or other annuity or periodic payments have started.”

Due dates in the instructions and on the form

The Instructions for Forms 1099-R and 5498 (2026) and Copy B print these dates, each quoted on this page: May 31, 2027 for filing with the IRS; February 1, 2027 for the statement to each participant; May 31, 2027 again for contribution information for all other types of IRAs, in the note on SIMPLE IRAs; and January 31, 2027 in the box 12b text, for a separate statement of the RMD amount. The instructions word the statement rule and the note this way:

“If you are required to file Form 5498, you must provide a statement to each participant by February 1, 2027. The statement must show the value of the participant’s account as of December 31, 2026, including information required to be reported in boxes 15a and 15b for hard-to-value assets and RMD, if applicable.”
IRS Instructions for Forms 1099-R and 5498 (2026), Specific Instructions for Form 5498, “Statements to participants”
“Note: Trustees of SIMPLE IRAs must also provide a statement of the account activity by February 1, 2027. Contribution information for all other types of IRAs must be provided by May 31, 2027. You are not required to provide information to the IRS or to participants as to whether a contribution is deductible or nondeductible. In addition, the participant is not required to tell you whether a contribution is deductible or nondeductible.”
IRS Instructions for Forms 1099-R and 5498 (2026), Specific Instructions for Form 5498, “Statements to participants”

When the account has no reportable contributions, the instructions give a separate rule:

“If you furnished a statement of the FMV of the account (including information required to be reported in boxes 15a and 15b for hard-to-value assets) and RMD, if applicable, to the participant by February 1, 2027, and no reportable contributions, including rollovers, recharacterizations, or Roth IRA conversions, were made for 2026, you need not furnish another statement (or Form 5498) to the participant to report zero contributions. However, you must file Form 5498 with the IRS by May 31, 2027, to report the December 31, 2026, FMV of the account and the FMV of hard-to-value assets.”
IRS Instructions for Forms 1099-R and 5498 (2026), Specific Instructions for Form 5498, “Statements to participants”
“If no reportable contributions were made for 2026, complete only boxes 5 and 7, and boxes 11, 12a, 12b, 15a, and 15b, if applicable.”
IRS Instructions for Forms 1099-R and 5498 (2026), Specific Instructions for Form 5498, “Contributions”

Boxes 1 and 10 count contributions made in 2026 and through April 15, 2027, designated for 2026. For a due date that falls on a weekend or holiday, the Reminders in the same instructions say:

“Due dates. If the filing due date in these instructions falls on a Saturday, Sunday, or legal holiday, you will be considered to have timely filed if you file by the next day that is not a Saturday, Sunday, or legal holiday. Legal holidays for this purpose are legal holidays in the District of Columbia or a statewide legal holiday where the return is required to be filed. Also, a leap year does not extend the filing deadline. Dates of February 28 in these instructions remain February 28 even in leap years; the due date does not become February 29.”

Boxes 11, 12a and 12b: required minimum distributions

The instructions give the filer this rule for box 11. The Instructions for Forms 1099-R and 5498 (2026) print the age in it as 73; this page repeats the passage as printed and does not apply any other age rule.

“Check the box if the participant must take an RMD for 2027. You are required to check the box for the year in which the IRA participant reaches age 73 even though the RMD for that year need not be made until April 1 of the following year. Then, check the box for each subsequent year an RMD is required to be made.”

The same instructions print the age range that goes with it. For the amount, see the RMD calculator.

“RMDs. A traditional IRA owner/participant must begin taking distributions for each calendar year beginning with the calendar year in which the participant reaches age 73 (after December 31, 2022, and before January 1, 2033). Frequently asked questions

What is Form 5498?

The IRS About Form 5498 page says: “File this form for each person for whom you maintained any individual retirement arrangement (IRA), including a deemed IRA under section 408(q).” The participant instructions on the 2026 form say the trustee or issuer of the IRA submits the information to the IRS to report contributions, including any catch-up contributions, rollovers, repayments, required minimum distributions (RMDs), and the fair market value (FMV) of the account.

When is Form 5498 due?

The Instructions for Forms 1099-R and 5498 (2026) have the filer file Form 5498 with the IRS by May 31, 2027. They also say a statement must be provided to each participant by February 1, 2027, showing the account value as of December 31, 2026, and a note on SIMPLE IRAs adds that contribution information for all other types of IRAs must be provided by May 31, 2027. Boxes 1 and 10 cover contributions made in 2026 and through April 15, 2027, designated for 2026.

Is Form 5498 the same as Form 1099-R?

They are separate forms that share one set of IRS instructions, Instructions for Forms 1099-R and 5498 (2026). The IRS About Form 5498 page lists Form 1099-R, “Distributions From Pensions, Annuities, Retirement or Profit-Sharing Plans, IRAs, Insurance Contracts, etc.,” as a related form. Form 5498 reports contributions, rollovers, conversions, recharacterizations, FMV, RMD information and repayments.

What do boxes 11, 12a and 12b mean?

On Copy B, box 11 reads: “If the box is checked, you must take an RMD for 2027. An RMD may be required even if the box is not checked.” Box 12a shows the date by which the RMD amount in box 12b must be distributed, and box 12b shows the amount of the RMD for 2027. The filer instructions say boxes 12a and 12b are provided for the filer’s use, and that the filer may instead provide a separate Form 5498 or a separate statement.

What is the difference between box 2 and box 3?

Box 2 shows rollover contributions made in 2026, and box 3 shows the amount converted from traditional IRAs to Roth IRAs in 2026. The participant instructions say box 2 does not include conversions done through a rollover contribution from a traditional IRA to a Roth IRA, which are reported in box 3.

Does Form 5498 say whether a contribution is deductible?

The Instructions for Forms 1099-R and 5498 (2026) say the filer is not required to provide information to the IRS or to participants as to whether a contribution is deductible or nondeductible, and that the participant is not required to tell the filer. For box 10, Roth IRA contributions, the participant instructions on the form say: “Do not deduct on your income tax return.”

Is a Form 5498 filed when there were no contributions?

The instructions say the filer need not furnish another statement (or Form 5498) to the participant to report zero contributions if an FMV statement was furnished by February 1, 2027 and no reportable contributions, including rollovers, recharacterizations, or Roth IRA conversions, were made for 2026. They add that the filer must still file Form 5498 with the IRS by May 31, 2027, to report the December 31, 2026, FMV of the account and the FMV of hard-to-value assets.
